Care Coordination Program

All active clients at Nashville Collaborative Counseling Center have the option to receive monthly phone support from one of our team members, at no additional cost.

The team member will review your progress in counseling, discuss barriers to therapy, and review strategies to support your treatment. We will also provide support by collaborating with your other care providers, as necessary.

The main goals of Care Coordination are to:

  • Identify your mental healthcare needs,

  • Communicate your needs clearly to your other healthcare providers (as needed), and

  • Provide safe, appropriate, and effective care.

What does Care Coordination cost?

There is no cost to clients for this program. Care Coordination is totally free.

Can I opt out?

Absolutely! You can stop services at any time. 

Who will call me?

Our lead for Care Coordination is Sandon Bull. He supports assigning clients to a Care Coordinator. The Care Coordinator who calls you will be one of our Interns. Their bios can be found here.

How does Care Coordination help me?

Care Coordination helps you by:

  • Checking in monthly on your progress,

  • Providing any additional support or referrals needed to community resources, and

  • Collaborating with your other providers to ensure you have the care you deserve.

We help you to advocate for yourself and to be an active participant in managing your healthcare and wellbeing.
